I drunkenly slept with my best friends boyfriend. I never told her because I didn't want to ruin her life (she's still with the guy). It still haunts me to this day.

Leave it where it belongs - buried in the past. That, or confess after they split when she probably thinks he's a dick. You know the old saying about girlfriends: there shall be no penis between us.
