WorkSafeBC, B.C.'s work safety agency, says Fox's production team is responsible for failing to ensure the safety of its star Dylan O'Brien, who was injured while filming Maze Runner: The Death Cure in British Columbia.

A report filed by Fox Productions, however, tells a different story.

O'Brien was filming the latest installment of the sci-fi film series on March 17 at the Cache Creek airport in Ashcroft, B.C., when was hit by a car and was rushed to a Vancouver hospital with serious injuries.

The film sequence O'Brien was involved in required him to move from one moving vehicle to another.

O'Brien was rigged to one moving vehicle while riding on the back of another moving vehicle ahead of it.

However, according to a redacted May 16 inspection report from WorkSafeBC, this arrangement was changed from the planned and rehearsed action sequence.

WorkSafeBC's report states that the transition from one vehicle to the other was changed from two sequences to one and that no meeting was held to review the changes with everyone involved.

The redacted Employer Incident Investigation Report that Fox filed with WorkSafeBC, however, presents an account differing from the WorkSafeBC report.

Fox said that the trailing vehicle, which O'Brien was rigged to, prematurely slowed down, causing it to separate from the leading vehicle he was riding.

The employer report states that "the scene was thoroughly rehearsed and successfully completed approximately five times prior to the accident". It also states that a safety meeting was held with cast and crew prior to the sequence being filmed.

WorkSafeBC has given Fox Productions a June 15 deadline to submit a report on how it has improved safety procedures.

As a result of O'Brien's injuries, production on the film was shut down. The film was originally scheduled for a February 2017 release but it has since been postponed to January 2018.
