Forensic Psychiatric Hospital a 190-bed secure facility that treats people who've been found not criminal responsible or unfit to stand trial due to a mental disorder.

The hospital also treats people who've been transferred temporarily from correctional facilities to be assessed or treated.

Last January, the Forensic Psychiatric Services Commission, which operates the hospital, was fined $646,302.88 by WorkSafeB.C in connection with two separate incidents of violence involving patients with a history of atttacking and injuring medical staff.

"The employer failed to conduct a violence risk assessment that included a consideration of previous workplace experiences, failed to establish policies and procedures to minimize the risk of violence, failed to instruct workers in those policies and procedures, and failed to inform workers of the risk from patients with a history of violent behaviour," WorkSafeBC states on its website. "These were all repeated violations by the employer. In addition, the employer failed to investigate reports of unsafe conditions and take necessary corrective action.

"Furthermore, the employer failed overall to ensure the health and safety of its workers. These were all high-risk violations."

The hospital is on the western bank of the Coquitlam River in an area known as Colony Farm.
