it's your hormones. at the time before menstruation certain ones increase to help with "fertilization" so it's like you're in heat. when fertilization doesn't happen, that's when you uterus sheds and you get your period.

My wife hasn't been frisky for 10 years so that makes about 120 periods as a quick approximation - so I'd have to say not all women!
