It took a phone call with a very on old friend to remember the fact that I had a speech impediment when I was young that I'm over now. They had given me a nickname based on it, and I realize that's how they are born -as a way to tease you- so people who make up their own nicknames (ie. hip-hop stars) are doing it wrong.
