That you could offend someone by offering them your seat on Skytrain.
Senior lady walking slowly with a cane, offered my seat and received a shake of the head, a frown and a loud “NO!!”
Happened a few times the past several weeks.
Won’t happen again, no more polite offers from me.

Keep offering your seat! She’s not the norm.

Agree with @Ignore her
Also, how are you offering? Some people are sensitive regarding respect/etc, this doesn't mean you did wrong, but just something to consider. "May I help you?" is a much better place to come from than a "Let me help you!" Was it a "Would you like a seat?" or a "Here have a seat." Though you may have been the most considerate person, then again, please know she's not the norm.
This goes both ways though, to piggy back your rant, I can't stand a handful of people who demand you get up so they can sit. Know the ones? Respect goes both ways. I seem to have to wear a badge that says "invisible disability" or else I'll feel the shame of people's judgement.

I usually just make eye contact and motion like I'm getting up. People shake their head or accept. It's a little less intrusive.

And I am actually flattered when offered a seat! I usually smile, thank them, and then tell them I am okay. However if they stand then I gladly accept..It makes me feel special so please continue young gentlemen... this city needs a little charm these days...

Keep offering! It’s still the polite thing to do!
